Authenticated as game used on September 5th , 2008 versus the Phillies at Shea Stadium(last year of Shea). Church went 0 for 4 in this game...also in the line-up that day..Reyes, Wright, Murphy, Beltran, Delgado, Luis Castillo, and, Brian Schneider. Mike Pelfrey started the game for the Mets.

The helmet is labeled on the back and front with "Church" name, and on the back with the MLB logo as well as the MLB hologram.
This Getty Image is from 2008 and there seems to be a match to a scrape on the left side of the helmet when compared to the image.
Church played for the Mets in 2008 and 2009.
The Rawlings helmet has a LOT OF USE -wear inside and outside. The helmet is not cracked.
